Liability Waiver & Release
I, the undersigned parent or legal guardian, acknowledge that participation in
UA Flag Football activities involves inherent risks, including but not limited
to physical contact, running, and contact with equipment, which can result in
injury.
Assumption of Risk
I voluntarily assume all risks associated with my child's participation in
UA Flag Football programs. I understand these risks cannot be eliminated and
I accept full responsibility for these risks.
Release of Liability
I hereby release, waive, and discharge UA Flag Football Canada, its employees,
coaches, volunteers, and affiliates from any and all liability, claims, or
demands arising out of or related to my child's participation.
Medical Authorization
In the event of injury or illness, I authorize UA Flag Football staff to seek
emergency medical care. I accept responsibility for any resulting medical costs.
Agreement
By signing below, I confirm that I have read, understood, and agreed to this
waiver on behalf of myself and my child.
Signed by the team's coach or representative.
